Send a written request, along with a check for $23.42 made payable to MBACD for each year requested. This cost covers copying ($.50 per page (30 pages in total)), postage and handling. The requested forms will be mailed to you within 30 days of receipt of request and fee by MBACD via certified U.S. Mail.
A copy of Application for Tax Exempt Status (Form 1023) is available by written request to address above. Please make check payable to MBACD in the amount of $41.56.
